License Reinstatements

The License Reinstatement Application form is a fillable PDF. Please download, complete & sign, and submit via email to us at with the required documents listed below. 

The Reinstatement Fee is $200 and may be paid through our online payment page with a debit or credit card. The Board will accept a check by mail, but this is not the preferred method of payment and will delay processing, as the application review will not begin until payment is received. Once a complete application packet is received we typically issue the license within 3-5 business days. You will be notified via email and sent a PDF copy of the license. 

Please note that if you have continued to practice in Nevada since your license expired, your application may require additional review by the Board.

Eligible Applicant(s)

SLP, AUD, or HAS who previously held a Standard License in Nevada that has been expired no more than three (3) years.  

Educational Requirements

Same as Standard License.

Certification Requirements

Same as Standard License.

Examination Requirements

Dispensing AUD & HAS: These applicants must retake both the written and practical examinations unless one (1) of the conditions below applies:

  • They passed the examinations within the past five (5) years; OR
  • They did not pass the examinations within the past five (5) years but are currently licensed and actively practicing in another state.

Continuing Education Required for Reinstatement

At least 15 hours in the last 12 months and directly pertaining to the profession in which the individual is licensed. You must list these hours in your application but do not need to attach documentation. More information may be found on our Continuing Education page. 

Required Documentation

  1. ASHA Certificate of Clinical Competence: SLP applicants, please be prepared to submit with your application.
  2. NBC-HIS Certification: HAS applicants, please be prepared to submit with your application.
  3. Verification(s) of Licensure in Another State: Board staff will attempt to complete verification online. If not available in your state, please request an official license verification to be sent directly to the Board – these may be accepted electronically.
