Reinstate Your License

A standard, provisional, or apprentice license to practice speech-language pathology, audiology, or hearing aid fitting and dispensing that has been expired no more than 3 years is eligible for reinstatement. After January 1, 2026, this will also include an assistant license. Any other license type, or a license expired 3 years or more requires a new application.

Please DO NOT start a new application, as the licensing system will flag the prior record. Instead, use the application linked below.

Eligibility & Requirements

Education

Same as required when the original license was issued; transcripts do not have to be resent.

Certification

None, regardless of whether they were required at initial licensure.

Hearing Aid Dispensing Examinations

Dispensing audiologists and hearing aid specialists may be required to retake both the written and practical examinations unless they either 1) passed the examinations within the past 5 years, or 2) are currently licensed and actively practicing in another state.

*After January 1, 2026, all audiologists may fit and dispense hearing aids without passing dispensing examinations and holding a separate endorsement. More information may be found on our Hearing Aid Dispensing Examinations page.

Continuing Education

Requires proof of continuing education totaling at least 10 hours in the preceding 12 months and directly pertaining to the licensed profession, with at least 1 hour covering ethics, cultural competence, cultural humility, culturally responsive practices, or diversity, equity and inclusion. For dispensing audiologists, at least 3 of the 10 hours must relate directly to the practice of fitting and dispensing hearing aids.

*After January 1, 2026, all audiologists may fit and dispense hearing aids and proposed regulations include related CE revisions. All CE hours must be listed in the application, but documentation is not required. More information may be found on our Continuing Education page.

Verification of Other Licensure

Verification of all current or past license(s) will be completed online by Board staff. If your jurisdiction does not offer online verification, please have official license verification(s) sent directly to the Board via mail or email.

Application & Submission

DOWNLOAD THE LICENSE REINSTATEMENT APPLICATION

  1. The reinstatement application form is a fillable PDF. Please download, complete, sign, and submit the form via email to board@nvspeechhearing.org with your required documents.
  2. Pay the reinstatement fee through our online payment page with a debit or credit card. The Board will accept a check by mail, but this is not the preferred method of payment and will delay processing, as the application review will not begin until payment is received.
  3. Once a complete application packet is received we typically issue the license within 3-5 business days.
  4. You will be notified via email and sent a PDF copy of the license.
Please note that if you have continued to practice in Nevada since your license expired, your application may require additional review by the Board.
