  • In an effort to minimize operating costs, the Board does not print or mail license certificates.

  • Upon issuance, manual renewal, or conversion of a license, Board staff will email you a copy of the updated license.

  • The online renewal application will allow you you print a copy of your updated license and receipt at the end of the application.

All licensees are responsible for notifying the Board of changes in the information listed below within 30 days of the change. Board correspondence will always be sent to the most recent address and/or email on file in our system.You may log in into your record here to update the following:

  • Address
  • Email
  • Phone
  • Employment
  • Name Changes. *Note, you must log in to start this update in your record, and send additional documentation to the Board. Please see our Report a Name Change page for additional instructions.

Adverse Actions

Per NAC 637B.042(15), licensees  must also report any of the following to the Board within 10 days of the action:

  • An action is taken against any license, certification, registration or other credential held by the licensee that was issued by another state or territory of the United States;
  • A criminal charge is filed against the licensee;
  • The licensee is convicted of a criminal offense, other than a traffic offense which is a misdemeanor that does not involve alcohol or controlled substances;
  • A civil action, including, without limitation, an action for malpractice, is filed against the licensee; or
  • A settlement or judgment is made in any civil action, including, without limitation, an action for malpractice, in any case filed against the licensee for any act relating to the practice of audiology, speech-language pathology or fitting and dispensing hearing aids.

HAS Apprentices (current) & SLP Assistants (after January 1, 2026)

Apprentices & assistants must ensure their records reflect current sponsor(s) & supervising SLP(s). 

Hearing Aid Specialists & Apprentices

Per NRS 637B.241, every hearing aid specialist and licensed apprentice must notify the Board of the new address of their place of business within 10 days after the change.
