Report an Adverse Action Taken Against You
What Must Be Reported
NAC 637B.042(15) requires licensees to notify the Board in writing within 10 days after any of the following occurs:
- An action is taken against any license, certification, registration or other credential held by the licensee that was issued by another state or territory of the United States.
- al offense, other than a traffic offense which is a misdemeanor that does not involve alcohol or controlled substances.
- A civil action, including, without limitation, an action for malpractice, is filed against the licensee.
- A settlement or judgment is made in any civil action, including, without limitation, an action for malpractice, in any case filed against the licensee for any act relating to the practice of audiology, speech-language pathology or fitting and dispensing hearing aids.
How to Report
Please send written notice of any adverse action listed above taken against you with in 10 days of the action, including as much detail as possible and all relevant supporting documentation. Notice may be sent to the Board by any of the following means, and should include the subject line: Attn: Executive Director - Adverse Action
