The applications below should only be used by the following:

  • An applicant who has never held a Nevada license; or
  • An applicant who previously held a Nevada license that expired more than 3 years ago.

Required Documentation

You may be required to upload one or more of the following documents in the online application, or have them sent to the Board office, as appropriate to your situation and the type of license for which you are applying:

Upload Into the Application

  • A current photograph (a driver’s License/ID card copy is acceptable)
  • Certification documentation from ASHA or NBC-IHS.
  • Military/veteran/spouse ID, DD214, or other documentation if reporting military affiliation.
  • Verification/copy of license(s) held in other jurisdiction(s)
  • Letter of explanation and supporting documents if answering “yes” to any legal/licensing history question
  • HAS Apprentice Sponsorship & Training Agreement (HAS Apprentice only)
  • Clinical Training Plan (SLP Assistants only – after January 1, 2026)
  • Letter of Completion temporarily in lieu of transcripts (New graduates only)

Sent Directly to the Board

  • College/University Official Transcripts (Not required for Standard Expedited License by Endorsement)
  • PRAXIS Score Report (Provisional SLP only)
